set your account to private, you can control if you want a "spammer" to follow you or not

Step 1
Use the Twitter Twerp Scan web page to calculate the ratio of friends to followers among your fans. Users that follow many people but have few followers are more likely to be spammers. This isn't always the case, of course, but it is one sign to watch.
Step 2
Observe the potential spammer's tweets. If her tweets are frequent, not conversational and always contain a link, she is probably a spammer.
Step 3
Navigate to the user's "Twitter" web page, once you've identified that user as a spammer. Click "Following" to stop following the spammer.Click "Block USERNAME" underneath the user's "Following" section to completely block the user.

You can't really, they search for keywords like what your last update was and then follow you. I'm sure the programmers at Twitter are working on fixing that problem.
