the icons on my desktop and my tool bar disappeared

At first, Mozilla Firefox didn't work, and i had to keep restarting my computer, a little after i realized that my tool bar, with the start menu wouldn't work, i've done virus scan's, spywear, and there is nothing, i used CCleaner to clean it out, still nothing, i want my icons and tool bar back please help...

oh and when the XP log in menu thingie comes up and i click on my name to get on, it says "userinit.exe application failed click OK to cancel application". please help with my computer.

First go to control panel then go to add or remove programs then look for firefox uninstall it and donloade it again
